Student of Concern form

 

Submitting a Student of Concern Report 

To submit a Student of Concern report, you can either email: studentofconcern@ucl.ac.uk or submit using the below form. If you have any concerns, we suggest you email the team. 

When submitting your report, please include the following information: 
  • Student name (including any middle or given names), 
  • Student number (this is the Student's UCL ID number), 
  • Student programme of study
  • Your relationship to the student (i.e. a lecturer, departmental administrator, or family member)
  • A summary of your concerns 
  • Whether you have discussed this concern with the student (and, if not, you would like us to tell the student the report came from) 
  • Whether you have any additional documentation you wish to share with us
