For Consultant to Consultant Referrals:

For GP referrals to the service via NHS e-Referrals System:

Please select "neurology", then "neuromuscular", then search by postcode as WC1N 3BG – the Queen Square Neuromuscular Service has an internal triage to allocate patients to particular clinics.  Please mention that the patient has FSHD to guide booking to the clinic.

As of now, a referral cannot be made by searching by the consultant name.
