The service improvement plan (template available below) identifies the priority goals for your team and the actions required to achieve these. We suggest that each CRT aim to identify 3 or 4 key service improvement goals to be achieved over a 12 month period. These should be clearly linked to specific, measurable items on the fidelity scale. We anticipate that potential areas for improvement will be highlighted from the fidelity review, but that priorities will be best identified locally and set in the course of the Scoping Day and agreed by the Service Improvement Group. It is important that the service improvement goals identified belong to the whole team, and are not simply the views of one or two individuals. They must also be realistic and achievable both within the study timescale and within the context of local resources.
